Dr. Prashanth Rawla, MD
Internal Medicine Specialist
Dr. Prashanth Rawla is a seasoned Internal Medicine physician with over 15 years of clinical experience. Board Certified by the American Board of Internal Medicine, Dr. Rawla is well-versed in managing a wide range of medical conditions, including diabetes, hypertension, and coronary artery disease. His clinical interests also extend to autoimmune diseases and gastrointestinal disorders.
Dr. Rawla completed his residency at Monmouth Medical Center in New Jersey, where he honed his skills in patient care and medical research. He is also an accomplished researcher, with numerous publications in leading medical journals. His work has significantly contributed to advancing the understanding of gastrointestinal diseases and various malignancies.

Dr. Azeem Mohammed, MD
Internal Medicine and Nephrology
Dr. Azeem Mohammed was born in India and obtained his medical degree at Deccan College of Medical Sciences. His residency was at Albert Einstein University and its affiliated St Barnabas Hospital, and he completed his training with fellowship at Augusta University in Nephrology. He stayed on as faculty and during his time there, he won several teaching awards, authored several papers and did pioneering work in life saving COVID 19 dialytic therapies.
Dr. Mohammed has over eleven years’ experience in Internal Medicine and Nephrology with a special interest in resistant hypertension and diabetic kidney disease.
As one of the founders of the MD Care Clinic, he believes individualized care and precision medicine will have a dramatic effect on the health outcomes of his patients.

Dr. Seema Faiyaz, MD
Board-Certified Gynecologist
With over 14 years of experience in women’s health, Dr. Faiyaz is a trusted and compassionate provider offering office-based gynecologic care for adolescents and women at every stage of life. Her practice is built on the foundation of personalized, evidence-based care, designed to support women through the many physical and hormonal changes they experience across the lifespan.
